Log Book Formats

There are two book formats available for the Driver's Daily Logs. "Format A" includes non-required items that "Format B" does not.  Choosing A instead of B, or vice versa is not critical, since both formats meet DOT requirements.

Book Format A

Book Format A includes these non-required items:

  • Total Mileage Today box
  • Home Terminal Address line
  • "From" line
  • "To" line

Book Format B

Book Format B not include these non-required items:

  • Total Mileage Today box
  • Home Terminal Address line
  • "From" line
  • "To" line
